You're probably hitting Cmd-; at some point -- this is bound to
toggle-mac-option-modifier.

I hit that inadvertently sometimes, when I'm trying for Cmd-'
(comment-or-uncomment-region-or-line)

You can correct it by repeating the same keystroke.
