[OS X TeX] LaTeX fails to find files

texhash doesn't take into account the files in your home directory at  
~/Library/texmf/: when running the TeX executables this directory is  
searched first, before using the database /Library/teTeX/share/ 
texmf.local/ls-R to search the /usr/local/teTeX/ tree.

In other words: there is no need to run texhash after putting files  
inside ~/Library/texmf/, these files will be found anyway.
