[OS X TeX] Problem with the Languages configuration in i-installer's TeXLive (starting this time a really new thread)

No, you did nothing wrong. A (potential) change in languages did not  
force a recompile of formats. The second time around, probably the  
"create formats" was checked and the previously changed settings were  
picked up.

I have fixed this in the TL i-Package and it will be in the next  
release.
