Do you have the fonts lbmdi.pfb and lbmdo.pfb in your path? For me:
$ kpsewhich lbmdo.pfb
/usr/local/teTeX/share/texmf.local/fonts/type1/yandy/lumath/lbmdo.pfb
$ kpsewhich lbmdi.pfb
/usr/local/teTeX/share/texmf.local/fonts/type1/yandy/lumath/lbmdi.pfb
You may also want to inspect these font files with FontForge (the X11
version), to see if all the glyphs are there.
Similarly, do you have the following lines in pdftex.map and
psfonts.map:
hlcdim LucidaNewMath-DemiItalic <lbmdi.pfb
hlcdima LucidaNewMath-AltDemiItalic <lbmdo.pfb

Thanks for the tip, I'll have a look later (no right now, I'm on
