[OS X TeX] APA style literature review

Greetings!

I have to write a literature review in APA style.   Something I have
never done before.

I am able to get the right heading with:

\pagestyle{myheadings}
\markright{Mathematical Representations}

However, I also need to have the page numbers top right, just to the
right of the heading.

I need to have the institution on the title page below the name of the author.

numbering starting with the title page,

no date on the title page,

and finally,

a running head on the title page, left justified, just below the header.

I use package apacite for my citations, so that is taken care of.
