[OS X TeX] Index: Balancing columns

Hello,

I use MakeIndex and would like to balance the columns in length on the  
last page of the index. I found out that the multicol package could be  
used, but I failed to achieve that.

I use a customized *.ist file as follows and would like to know how I  
could include a balancing into that *.ist file.
