Howdy,

First, TeX Live 2009 works fine with Leopard; no need to update to Snow Leopard because of that.

Second, tlmgr is an application that first came with TeX Live 2008 and allows the user to keep TeX Live up to date as packages, etc., get updated during the year (or so). That way you can get bug fixes (and new bugs too :-)). It also allows you to control certain parameters (e.g, default paper size assumed by some of the compilers [note: LaTeX always assumes letter paper by default independent of the chosen paper size assumed by the processors]). 

The TeX Live Utility, which comes as part of MacTeX on Leopard and Snow Leopard installs, is a really nice GUI front end for tlmgr.
