You definitely have tlmgr from TL 2008, but it's installed in /usr/local/texlive/2009. I have no idea how this could happen. I'd suggest
cd /usr/local
sudo rm -rf texlive
to remove your current install and any previous ones, then reinstall via MacTeX. You could also check `ls -l /usr/texbin` to make sure it's really a symlink.
