[OS X TeX] hyperref index and \CodelineIndex

I'm using hyperref to generate hyperlinks in my index when I process my dtx file. Everything works fine if I use \PageIndex in my preamble, but if I use \CodelineIndex there are no hyperlinks in my index. Is there a way around this? I've checked hyperref's documentation and searched comp.text.tex but have found no references to this problem.
