We others have the new one from 2011/08/28 v3.0k,

we're not using that CFG file,

our's is from 2011/06/27 v1.7a,

our DEf's are from 2011/08/30 v6.4m.

You need an update of eqexam_pack.zip! http://www.math.uakron.edu/~dpstory/eqexam.html (And also check which multicol.sty gets used, an old one from the not updated TeX Live 2011 or some private copy. This is recorded in the LOG file.)
