Really ? Have you used it recently ? The app one can download just now is version 2.0.4 from 2006. In its web page, the developper has a warning that the program will not work under Snow Leopard ! No mention is there that it will work again in Lion nor in Mountain Lion.

I too used to use it long ago. I have just tried with Lion now and it does not work (neither as a Service nor as a PDF option in the Print dialog).

\usepackage{booklet} works fine for me.
