I ended up using DEVONtechnologies' WordServce, which had been recommended by several people on this list, to do a count of the resulting PDF.
I don't think there is any ``pre-processed'' word count program that actually goes after the bibliography. I'd guess you need something that analyses the final PDF to get that right.
