[OS X TeX] Continuing index subentries across pages

Hi, everyone.

I have yet another (I hope last ... for now) index question.

In the Index there are entries at the bottom of page 533 (right column)

  Number theory, 197
        abundant number, 251
        aliquot parts, 248
        amicable numbers, 252

and the subentries continue on page 534 (left column) with

        commensurable magnitudes, 162
        commensurate, 222

&c.  My question is, how can I insert a "heading" at the top of page 534 so that it appears as

  Number theory (continued)
         commensurable magnitudes, 162
         commensurate, 222

&c.?  Thanks in advance.
