[OS X TeX] font issues with lucida under MacTeX 2017

Additional information:

(1) For the source shown, file mktexpk --mfmode / --bdpi 600 --mag 1+0/600 --dpi 600 hlhr8r consists of:

   mktexpk --mfmode / --bdpi 600 --mag 1+0/600 --dpi 600 hlhr8r

(2) My ~/Library/texmf/web2c/updmap.cfg includes the line:

     Map lucida.map

and ~/Library/texmf/fonts/map/dvips/lucida/lucida.map includes the line:

     hlhr8r LucidaBright " TeXBase1Encoding ReEncodeFont " <8r.enc <lbr.pfb
