Notice that the path to the files is /Users/kannan/Library/texmf/tex/latex/... so it is using files from your personal texmf folder rather than the one from your distribution. That file is probably out of date so please remove it.

That worked like a charm. Thank you very much. I tried everything else earlier, and spent quite a bit of time, trying to figure out, but couldn't.
Perhaps this can be mentioned in a read me file - to remove the personal files. This will especially be useful to people who have been using LaTeX for many years, and have possible gone senile :)

Howdy,
The real answer is to never put `updated' packages in your personal tree. It's meant for personal packages, etc., that are not part of TeX-Live. Any time I put something from CTAN in that tree I place it in a special directory that I check when I update TeX-Live to make sure it hasn't been added to TeX-Live.

<p>That is precisely what I did. These are all packages that were
not included in the standard distribution at one time, listings,
and ccicons, for example (I did not create any package of my
own). Perhaps I should have kept them at some other location.
The main issue is that I did not check whether these got included
in the standard distribution, and if so, remove them. I will
follow your advice from now on.<br>
