Try putting it into ~/Library/texmf/tex/bibtex/bst/misc/
Works for me:

 > kpsewhich plain-annote.bst
/Users/will/Library/texmf/bibtex/bst/misc/plain-annote.bst

texhash is not necessary for items added to ~/Library/texmf/...
