This is not true:
ls -l /usr/local/tetex
total 88
lrwxrwxrwx 1 root admin 27 1 Jan 1970 README.* -> share/texmf.macosx/README.*
...
Only for filename completion and globbing it is case sensitive.
ls -l /usr/local/tet*
ls: /usr/local/tet*: No such file or directory

No, it's the file system itself. And you can have both case sensitive and
case insensitive filesystems at the same time AFAIK.
