This seems to be very relevant, given nobody else here has been able to 
reproduce the bug that you experience, using your former example:

	\[ \begin{CD}
	1 @>>> 2 \\
	@VVV @VVV \\
	3 @>>> 4 \\
	\end{CD} \]

Can you provide a really minimal example which, processed on its own, 
triggers the bug?

It may also be helpful to search your hard drive (using the Finder's 
search capability, for example, after specifying to search for visible 
and invisible files) for the files pdfsync.sty and amscd.sty, in case 
there are duplicates inside your ~/Library/texmf.
