I'm in the process of reinstalling TeX from scratch right now, to
switch to Gerben's new setup. Hence I cannot test your example until
that is finished, and given I'm on home ADSL that might not be until
sometime much later today!
Looking more carefully at your example, I can but guess for now:
- Maybe \protect\index instead of \index.
- Maybe load the hyperref package last, after makeidx (as a rule it's
better to always load hyperref last, as it redefines internals of
lots of other packages, most from the base LaTeX distro but some also
out of this base distro).
Still, as you said, that would in any case signal a bug.
