Just declare your own IndexAndMarginNote-Command, maybe you should also
have a look at the new marginnote package by Markus Kohm decreasing the
side effects of marginlines significantly.

\newcommand{\IndexAndMargin}[1]{%
\index{#1}\marginnote{#1}%
}% -- untested --
