[OS X TeX] Help hyperref causing errors

Thanks for the help, I should have tried to narrow it down right away,
but I was hoping it was something easier.  It turns out that it is
some incompatibility between the book style file I got from my
publisher, hyperref, and the listings package.  I'm blaming it all on
the ancient style file from the publisher.  Hopefully he will have an
updated one for me.
