[OS X TeX] Old file version reappeared as ghost

A quick check to verify that the files are what you think they should be is to CMD-click (hold down the 'Command/apple' key while clicking and holding the mouse) on the icon on the title bar next to the file name.

That should drop down a list showing (in reverse order) the full path to the file in question.

If nothing else, this will tell you with certainty what files you are using.
