Howdy,
1)As far as the Icon problem is concerned... have you used Onyx (the version for your OS) to rebuild the LaunchServices database? That usually clears up that problem although a reboot directly afterwards may help (how I dislike saying things like that!).
2)Where are you not seeing the .tex extension? If in Finder windows please make sure you check the Finder->Preferences->Advanced->Show all filename extensions option.
3)If you are using OS X 10.8 make sure you have the Close windows when quitting an application option checked in System Preferences->General. If the file is an unsaved file it will open automatically no matter what your setting. Under OS X 10.7 the option is stated in a different manner but it's there. PS: I'm not sure what you mean by ``application expose''.
