[OS X TeX] MacOSX-TeX Digest, Vol 87, Issue 10

Googling reveals that the most likely culprit is a conflict between the hyperref package and another package. First try to call hyperref as the last command of the preamble, second, if this doesn't help look at the log file to see which package might conflict with hyperref or load the other packages successively till the conflicting is found.
