Run these two commands in the terminal, reboot in safe mode and then reboot normally. This worked reliably for me in Yosemite. 

ons/A/Support/lsregister -kill -seed

sudo find /var/folders/ -name com.apple.dock.iconcache -exec rm {} \;

Version 2.8.0 (2.8.0)
