It is possible that this is being done in the background.  I have in the
past had problems where ps libraries (ps2pdf? dvips?) have the a4
settings baked into them.  On Linux, I ended up having to troll around
in /etc/ to fix system configuration files.  Perhaps there's something
similar for you on OS X.
