I am using it, and it has exactly the same fmtutil.cnf that you are 
quoting. No *.efmt though.

I have given my advice in an earlier message to this list (sent more 
than 12 hours ago), namely to run

   texhash
   sudo texhash

but I don't seem to be allowed to post to the list.
